Community Assessment and Plan of Care

Assessment of a community is an important process by people working in the public health department including nurses that involve identification of the assets, strengths, challenges, weakness, and needs of a community while understanding the community better and therefore coming up with an effective care plan that promote and maintain health of the community (Pennel et al., 2017). The primary goal of this activity is to address the health needs including other identified needs of the community. Results from a community assessment can be used for community health improvement or care plan. This is a systematic and long term effort that is designed according to the needs of the community to address public health needs and challenges to promote health status of the community (Pennel et al., 2017).
